Hair is a seldom-used specimen for detection of theroin derivatives in most laboratories in Thailand but its analysis has the potential of making a valuable contribution. Therefore, this study is set up in order to determine an effective extraction method, which can be used in laboratories in Thailand. It was found that 50 mg of hair sample extrated with 5 ml of methanol at 60C in waterbath for 16 hours provided maximum heroin derivatives concentration. The method involving methanolic extractions of the drug followed by analysis with the immunoassay TDx system is capable of providing drug identification as well as accurate queantitation. The limit of detection LOD for drugs in hair was applied to tests of hair from non-users of drugs. According to the result obtained, values of drug concentration above 0.01ng/mg were considered to be a positive result. This study also compared between heroin derivatives in human hair and urine in 70 heroin users. Sixty subjects were randomly selected from patients at the drug addiction clinic of the Bangkok Metropolitan Administration Medical College and Vajira Hospital and the rest were collected at postmortem examination from the Department of Forensic Medicine, Faculty of Medicine Siriraj Hospital and the Institute of Forensic Medicine, The Royal Thai Police Head Quarter. There were 27 cases 38.57% whose test for drugs in urine were negative but whose hair samples were positive and only one case 1.43% whose test for drugs in urine were found to be positive but whose hair sample was negative. Because the urine test is limited for only recent drug usage while the hair test is able to demonstrate the past, and chronic drug intake, sample selection for drug analysis should be carefully considered.
