The authors began stereotaxic amygdalotomy in 1961 and up to Feb 1966(a period of 5 years) 90 operations had been performed. Among these 36 patients were operated upon 51 times with the object of alleviating mental symptoms. 22 patients had unilateral surgery 10 had bilateral operations, 3 had two operations dome in the same side, and 1 patients underwent three surgical procedures-one in a side and two in another. The diagnoses in the series included Inteructal psychosis (22 cases). Hyperkinetic behavior disorder, schizophrenia with olfactory hallucination, Lctal psychosis, Posttraumatic personality disorder and Obsessive-compulsive neurosis. Preoperative preparations included scalp and sphenoid EEG. Psychological test were done in every patient when feasible. Candidates for surgery were selected from the above diagnoses with temporal, particularly anterior tempora;, focus or foci of EEG abnomalities such as spikes or sharp waves. Steriotaxic amygdalotomy was performed by using Cooper basal ganglia guide and Rand-Welle's stereotaxic intrument. Production of lesion was done by injection of iodilax and later by cryogenic method. During surgery scalp EEG and smygdalography were recorded. Postoperative scalp and sphenoid EEG were repeated. Psychological tests were done 1 month and 1 year after surgery. Of the 51 operations 6 were unsuccessful due to development of cardiores-piratory arrest when the patient was sit up after a trephination was made. It was believed that this was due to brain stem ischemia resulting from compression of the basilar artery against the clivus when the brain volume moved forward after the cranial cavityn lost its closed-box nature. One patient had cardiorespiratory arrest, was successfully resuscitated, but redeveloped the failure 17 hours later and expired. The result of amygdalotomy upon mental states in cach category would be described in detail in the next chapters.
