Moclobemide, a reversible inhibitor of monoamine oxidase (RIMA) appears as effective as a tricyclic in treating various forms of depression, but to the author's knowledge, there is no report of moclobemide in the treatment of magor depression with panic attack and obsessional thought. A 29-year-old woman who met the DSM-III-R criteria for major depression joined an open study for clinical trial of moclobemide. She was also diagnosed as having panic aattack and obessional thought and was given 100 mg of moclobemide thrice daily. At the end of week 6, her depression symptoms were much improved : there was an 70% reduction on the 17-item Hamilton Depression Rating Scale (HDRS) score from the baseline. Severity of symptoms which were rated by the patient's self assessment and psychiatrist's assessment by using visual analogue scale(VAS) also showed much improvement (from 6.7 and 6.1 at baseline to 1.8 and 1.2 at the end of week 6). Surprisingly, her obsessional thought also disappeared and her panic attack was much improved to the level of limited symptoms attack. It was recommended that a prospective, double-blind placebo controlled study of moclobemide in the treatment of primary anxiety disorders (panic disorder or obsessive compusive disorder) should be carried out.
