The objectives of the current study were to develop a model of comprehensive psychiatric care, examine the Knowledge of health personnel, village health volunteers (VHV) and psychiatric patients relatives concerning psychosis issue such as therapeutic and home-based care in community setting as well as the referral system, and to reduce the restrained rate. This project was conducted in 200. Considering the health personnel's readiness and the great amonunt number of restrained patients, Amphur Varitchaphum was selected sample for the pilot study. Twenty two of health officers, I.e. health personnel, registered nurses, technician nurses who were in charge of mental health task in health center and hospital setting 65 VHV (one from each village). And 62 patients' relatives (one from each family) were trained on how to provide care of patients. In addition, there were home visit network from VHV through provincial level. Group conference and home visit were provided by health personnel, VHV and patients' relatives. The evaluations were conducted at 3 and 6 months afterward. Frequency percentage and t-test were used to analyzed the data. The results that the knowledge of health personnel and VHV after trained were sigificantly increased six of ten restrained patients were released. The therapeutic care and referral system were well established. Moreover, the patients recore was innovated for the sake of communication between patients' relatives and physician. Six patients were screened and referred by VHV for further investigation: 4 patients were referred by their families for injection, and 6 patients were referred to Khonkhan Mental Health Hospital. The provincial survey of restrained psychiatric patients held in 2001 found that there were 62 restrained psychiatric patients. Amphur Arkadamnuoy and Kusumal which selected as extended area of study were followed the same procedure as in 200. In conclusion, the project can reduced restrained rate. 27 patients were released. The comprehensive psychiatric health caare can decrease the restrained rate.
