A case of Capgras syndrome associated with organic brain syndrome with psychosis due to cerebral artereosclerosis is reported. The patient believed that her eldest son was an impostor who had assumed the exact appearance of her real son. The psychotic manifestations included auditory hallucination, delusion of persecution and the Capgras syndrome in the setting of clear consciousness but with marked impairment of memory. The phenothiazine was given with much improvement of her psychotic state but the Capgras syndrome wasunchanged. It is apparent that this syndrome can occur in a number of different psychiatric disorders including organic brain syndromes.
