Sexual problems have been chronic problems in Thai society. Sex has not yet been discussed as openly as other subjects, because it involves our deep personal feelings and the cultural belief. Consequently many people are confused and bothered by their sexual feelings. Sexual problem,s seem to be on the increase. Many cases of chronic sexual inhibition are due to faulty learning and the majority are the result of conditioning. Usually anxiety responses have necome conditioned to the stimuli associated with sexual responding and being incompatible with the latter, inhibit it. The same inhibitory effect may be due to other conditioned emotions, like shame or disgust. A young female patient came to the clinic with her chief complaint tha she had an obsessional fear of being raped by men. Her early history was quite conventional. Her problem was due to early sexual experiences that had attached negative and anxious feeling towards sexual stimuli. The problem developed after an attempted rape by her cousin and other sexual trauma. Her repeated anxiety had created a growing negative obsessional idea towards sexual activity. The treatment of sexual problem depends upon what the stimulus-response analysis of the case reveals. To re-educate the patient and to remove misconceptions about sex is frequently not sufficient, because most patients are still left with a negative emotional attitude to sex, bound up with anxiety towards various aspects of the sexual situation. The treatment of behavior therapy is generally systematic desensitization determining by the identified stimulus antecedents of the anxiety. Systematic desensitization is the main technique which was used with this patient. The method involves two components. Firstly, the patient was trained to be relax in order to counteract anxiety. Secondly, a hierarchy of anxiety-provoking stimuli was made up, rangins from items which caused mild anxiety, to those which caused severe anxiety. After 23 sessions the patient reported that her life style had changed radically. She would now go anywhere without the anxiety which had been associated with sex stimuli and felt that she could enjoy life to the full. The improvement was maintained at the 10 month follow-up, when the patient described her attitude to the males as being very much akin to that of a normal person. On my personal experience, behavior therapy is a valuable method of treatment for sexual problems.
