The objectives of the study were (1) to describe the mental health status and the defense mechanisms of Thai inmates, (2) to identify the correlation between the inmates' mental health and their defense mechanisms, (3) to compare the mental health, the defense mechanisms by sociodemographic characteristics, and (4) to compare mental health and defense mechanisms by the offenses profiles. The study was done in the prisons and jails distributed around Thailand. The total study population was 5,303 inmates, which the number of male inmates was 4,313, while the female inmates were 990. The stratified systematic random sampling was used. The questionnaires consisted of the sociodemographic characteristics, the Thai Mental Health Indicator developed by Apichai Mongkol, et al (2001) and the DSQ-40 (Thai version) modified by Chaiyos Jittirungsun (2001) were employed as the study instrument. The statistical test as frequency, mean, standard deviation, median, Chi-Square test, Kruskal-Wallis test, and Pearson Correlation Coefficient test were used for data analysis. The study results were found that fifty six percent of the inmates had poor mental health. They used mature defense mechanisms more than neurotic defenses and immature defenses in orderly. Their mental health status had positive correlation with the mature and neurotic defenses , while it had negative correlation with the immature defenses. However, the level of all of the correlations was low. The percentage of the poor mental health among the younger and lower educated inmates was higher than the compared groups. The sentenced prisoners or the first convicted to offence inmates or the inmates who had only one case had better mental health compared to the other groups in the same profiles. The female inmates use all of mature, neurotic and immature defense mechanisms more than the males. The elder or the higher educated or the first convicted to offence inmates used higher frequency of the mature defense mechanism. The illiterate used neurotic defenses more than the educated inmates. The study was beneficial to understand the inmates' mental health and their defense mechanisms used. This basic information can be set the benchmarking in monitoring the mental health care development in the prisons. The inmates' defense mechanisms changed to be the higher level can be used as one of the inmates' correctional goal.
