In-patient drug dispensing system in Suansararom Psychiatric Hospital has been change from dispensing per prescription to medication profile system in 7 patient wards. In which the later system, pharmacists copy the medications that the doctors prescribed from "Doctor Order Sheet" to the patient medication profile which the records of previous drug used by patients were already written. This study had 2 purposes. One was to compare the medication error of the prescription system and medication profile system in 30 subtypes: transcription error, pre-dispensing error and dispensing error. The last objective was to find out how much does the cost of drug in medication profile system returned to pharmacy division. The data were collected from medication error record and the medication cost record from March 1st 2003-May 31st 2003. Independence t-test was used to test the different of medication error of two systems at 95% confidence interval. The cost of drug returned was reported in Bath with median and IQR. As the result, the errors from 2 systems were mot significant difference except the pre-dispensing error in typing category. The typing error from medication profile system was 3.85% and the prescription system was 1.73% . The mean difference was 2.12% (p-value=0.035, 95% CI=0.25-3.99). From the data of 455 patients, cost of drug returned to pharmacy division was 94,019 Bath (median,IQR=15.00,97.00). The result of the study gives us the idea to improve the medication profile system, such as modifying medication profile form format, so it would be easily for the typing clerk. Value in saving cost from the drug returned to pharmaceutical Department is the background data. We can discuss the cost as the medication profile system is convenient to collect cost value while the previous system is not so it was presented as high returned drug cost, but what is the background behind returned drug number. If we could reduce the number of drug returning to pharmacy division, officer's work load would reduce also and minimize the problem about drug's stability when medication were outside the temperature control room. However in medication profile system, pharmacists detected 31 risk events from total 35 events. Risks that are usually found included home medication in OPD card are not corrected, drug were sent back to Pharmacy division too early (before duration of treatment ) It is interested that how many risks occurred in the prescription system that we could not discovered them. It is worth to implement new system in other wards because it will reduce the number of medication error risks.
