Background and Significance of the Study : There are possible cardiovascular serious complication in the patients who used psychotropic drugs because of the interactions of drugs with local anesthetics in dental procedures. The risks should be identified for patient's safety. Objective : The purpose of this study was to determine cardiovascular effects and drug interaction do local anesthetics with or without vasoconstrictor and psychotropic drugs used in psychiatric inpatients. Method : The samples were 41 psychiatric underwent dental treatment at Galya Rajanagarindra Institute during January 1st to June 30th, 2002. The arterial blood pressure, pulse rate and observed symptoms at pre-and post-operation were recorded. The statistics used were frequency percentages, cross-fabulation and paired t-test. Results : Almost patients received anesthetic with epinephrine 1 : 100,000. There was no statistically significant change both of systolic and diastolic blood pressure, and pulse rate except group of patient who received epinephrine ‹ 12 Ug had significantly increased in diastolic blood pressure at p_<0.05 Postural hypotension occurred in one patient (2.4%) Who received 450 mg of chlorpromazine equivalent per day. Two patients who got 800-1000 mg (4.9%) of chlorpromazine equivalent per day, had dizziness symptom. Eighteen microgram of epinephrine was used in all of the mentioned cases. Conclusion : The dentists should consider the cardiovascular risks in the patients who received Chlorpromazine equivalent doses of 450 mg or above per day when administer the 18 Ug of epinephrine in the local anesthesia.
