A Thai male, aged 17, was admitted to Somdet Chaopraya Hospital because of mental and memory disturbances. A diagnosis of schizophrenia, catatonic type was made; he received an appropriate psychiatric treatment without benefit. He became more childish, emaciated and helpless. Death occurred approximately one year after onset of the illness. At autopsy, the brain showed a marked a marked loss associated with gloss in certain nuclei of the diencephalons and extra pyramidal system. The diagnostic difficulties and clinic-pathological correlation are discussed.
