Opium and heroin are two principal drugs of dependence in the northern region of Thailand. The drug dependents lowlanders and hill tribes use these drugs either smoking or intravenous routes. The serum from 45 intravenous drug users IVDU admitted to the Northern Drug Dependence Treatment Center NDTC on 15 March 1986 were tested for HIV infection by ELISA technique which resulted all seronegative. For serosurveillance, the serum of all admitted dependents were continuously tested. The first seropositive with Western Blot Confirmed was reported on 30 August 1988 that he was a lowlander who use intravenous heroin. One day later, the first hill tribe HIV seropositive was found. He was also the heroin intravenous user. Until 30 September 1990, 537 cases among 4,132 (13.0%) were found to have seropositive. Based on type and pattern of drug use, classification of the treated dependents showed significance differentiation that the highest percentage of seropositive were the heroin iv. And opium iv. At 43.0% and 20.4% consecutively. The lowest was 2.4% for the opium smoker. Among 537 seropositive cases, there were 84 hill tribal people, 69 men and 15 women. Karen was the majority followed by Yunnanese and Lisu at 29, 14 and 13 respectively. There were only 29 IVDU of both opium and heroin in this group. The percent study shows a wide spread problem of HIV infection among the hill tribes and tries to link some demographic and etiology of the problem which not only the derive from intravenous risk. The finding also suggests of influence from the changing on type and using pattern of narcotic drug which link with the HIV spreading.
