Clozapine an atypical antipsychotic drug, is only 30-60% effective in patients who resist to treatment with other antipsychotic. The most important adverse effect of this drug is agranulocytosis and death from infection. In this retrospective study, factors associated with agranulocytosis following clozapine administration at Somdetchaopraya Hospsital were estimate in 273 patient,s records. Patient's records after administration of clozapine for 18 weeks, the data of concentration of white blood cells, neutrophil, and blood platelets was collected and the granulocyte count were determined. Statistical analysis showed that percentage of the patients who had agranulocytosis, neutropenia leukopnia, was 0.7, 2.0, and 2.6 respectively. Sex, age at the beginning of clozapine administration, use of clozapine alone, use of clozapine with another drugs, unregularly of clozapine using and lack of following blood count did not associated with agranulocytosis. But the intersting point in this study was that, there were neutropenia and leukopenia 2.2% and 2.6% respectively, they may lead to agranulocytosis if the patients did not have a good warning on adverse effect. Although this study indicated that the incidence of agranulocytosis of patients at Somdetchaopraya Hospital was not striking, further study should be made to gather more data to determine the association between risk factors and agranulocytosis. Such investigation has to be done completely and accordingly to the safety blood monitoring of this drug usage in order to reduce the agranulocytosis in these psychotic patients.
