The objective of this article was to present the project "Continuity of Care for Enhancing Patients Adaptation to Breast Cancer and treatment." With the collaboration of nursing education and nursing service, this project was oraganized to promote quality of care for patients with breast cancer. Holistic health and continuity of care were used as conceptual basis for the project. Research-based nursing practice was incorporated in three major phases: 1) pre-admission phase; 2) hospitalization and discharge planning phase; and 3) post discharge and follow-up phase. The outcomes of the project were presented in terms of patients satisfaction with care nursing personnel development, and the resourcefulness of the project.
