The increasing numbers of elderly patients, chronic illnesses with complications, end of life care at home, cost containment and shortened hospsital stays are some of the challenges faced by hospitals today. The quality of discharge planning and home care are essential strategic plans to provide quality care and qualified continuing care to serve Thai health care needs. Home care teams have recognized the benefits of using specialty nurses to deliver quality care. Several studies have shown the benefits of accurately indentifying patients who need comprehensive discharge planning and home follow-up. Adequate discharge planning for elderly adults has resulted in a 14.8% reduction in nursing home placement, and significantly decreased the chance of unplanned hospitalization. Coordination plans between home care nurse and hospital personal may break the cycle of repeated hospitalization. Several other researchers have demonstrated similar positive outcomes when elderly patients were appropriately identified and their post-discharge care needs were met. Thus, it is time for the leaders of the nursing profession to think about how to respond to the patients' and families' demands. Preparing professionals who have advanced nursing knowledge and practices to provide care for the complicated needs of patients and families at home should reform home health care. Sooner or later, we will realize that the world has changed and that health care must change with it; home care is destined to become the very heart of health care.
