Chronic intractable pain of unknown etiology has always presented a major diagnostic and treatment difficulty in medicine. In Psychiatry, the role of depressive illness as the underlying cause of pain is well recognized. The diagnosis of depressive illness, however often poses a problem as many of these patients totally deny psychological problems or depressive mood changes. Applying the concept that pain ‘masks’ depression, the authors use modified Research Diagnostic Criteria, substituting pain as a mood equivalent for the identification of major depressive illness presenting as chronic pain. They report two patients with chronic intractable pain who has met this modified Research Diagnostic Criteria for major depressive illness. Both patients responded well with alleviation of pain symptom following treatment with amitriptyline. The authors discuss the use of this modified Research Diagnostic Criteria as a diagnostic tool to ensure a more accurate and prompt identification of major depressive illness presenting as intractable pain.
