The authors studied complications of heroin injection in 25 heroin addicts hospitalized between 1978 and 1980 at Siriraj Hospital. Both medical and socioeconomic complications were revealed. It was found that severe medical complications were as follows:- 1. Bacteremia:- staphylococcal bacteremia, pseudomonas bacteremia. 2. Acute fatal reaction:- coma and acute pulmonary edema. 3. Lung abscess and tetanus. 4. Cellulitis and abscess. The heart, lungs and kidneys were among the most important organs that were involved in bacteremia. The pulmonary complications were characterized by lung abscess, pyopneumothorax, emphysema, pleural effusion whereas endocarditis, heart failure were the cardiac complications in our patients. Visually even in the absence of audible murmur, the physician should treat the heroin addict with bacteremic complication as one who has endocarditis. The renal complications in our study were glomerulonephritis, acute tubular necrosis and renal failure. Pseudomonas bacteremia was the most serious complication with one hundred percent mortality. Without prompt and appropriate treatment, those with coma and acute pulmonary edema could rapidly become fatalities. Treatment of complications requires high quality antimicrobes, a great deal of investigations and several weeks of investigations and several weeks of treatment program at high cost. The authors recommended that early treatment of the non complicated cases and prompt and proper management of the problem of addiction are among the most valuable means in lowering the incidence of these preventable diseases. In addition, the attitude of the physician towards the heroin addict as an individual has great bearing on the success, or otherwise, of the treatment.
