รายละเอียด / Details:

Presently, family care-giving is crucial for psychiatric care due to the benefits of enhancing the patient to stay longer with the family and to decrease the relapse rate, wa well as the cost of care. The purpose of this study was to test the effects of a psychiatric family care-giving program on the care-giving skills of family caregivers who care for a person with mental illness at home. The target population of this study was primary caregivers who provide care at home for family members with mental illness. A convenience sample of 60 subjects was randomly assigned to treatment and control groups, with 30 subjects in each group. A total of 44 subjects completed the study; 21 in the treatment group and 23 in the control group. The control group received the regular care provided by a psychiatric nurse at a mental health clinic; the experimental group received a psychiatric family care-giving program. Both groups were tested for level of care-giving skills immediately pre and post intervention and at 1 month following the intervention. Family care-giving skills were measured by the Chiang Mai Psychiatric care-giving skill scale (CPCSS) developed by the researchers. The CPCSS is a 50-item likert-type scale that asks caregivers to rate their skills in carings in caring for their family member with mental illness in various situations. Validity for the CPCSS was 0.92 and internal consistency reliability measured by conbrach's alpha was 0.88. Data were analyzed by T-test and Mann whitney Teat. There were no significant differences between the two groups in sex, marital status, psychiatric diagnoses, length of caring income, or type off family. However, caregivers in the control group had a higher educational level. There was a significant difference (p=.00) between the two groups in length of illness of the mentally ill family member. Caregivers in the control group had taken care of the patient who had longer illness (mean=9.3 years, S.D.=11.69), while caregivers in the experimental group had to take care of family members who were actuely ill (p=0.04). There was a significant difference between pre and posttest mean scores on the CPCSS for the experimental group at the immediate posttest period (t = -2.35, p = .02) and 1 month follow-up (t = -3.31, p = .00). There was no signigicant difference between pre and posttest mean scores on the CPCSS for the control group I,,mdiate following the intervention. However, at the 1 momth follow up, there were significantly lower (t = 3.21, p = 0.00). When comparing the mean scores between the two groups, there was no statistically significant difference at the immediate pre and post experimental period. However, at the 1-month follow up mean scores for the experimental group were significantly higher than the control group (t=-3.51, p=.00). These data suggest that providing a psychiatric family care-giving program could enhance the care-giving skills of family caregivers for psychiatric patients. Caregivers who received the program in this study were able to maintain their level of care-giving skills 1 month post intervention, while caregivers who received regular care demonstrated a decrease in their care-giving skills at 1 month. These results suggest that a family care-giving program can enhance the ability of family caregivers to provide care for mentally ill members in the community.
