The objective of this study was to determine unit cost of mental health service at Nitijitavej Hospital from provider perspective. The retrospective study was done from October 1, 1999 to September 30, 2000 using record from of labour cost material cost capital cost and full time equivalent FTE concept. All departments were categorized into 3 cost centers of non-revenue producing cost center NRPCC, revenue producing cost center RPCC and patient service PS and cost was allocated from NRPCC and RPCC to PS by using the simultaneous equation method with appropriate allocation criteria. The results revealed that full cost of mental health service in Nitijitavej Hospital was 49,727,235.71 Bath with proportion of labor cost, material cost and capital cost being 71.62%, 14.58% and 13.80% respectively. The operating cost was 42,813,100.66 Bath or 76.20% of full cost. The total amount of in-patient service was 2,281 cases or 64,827 hospital days with the average hospital day of 28-42 hospital days per case. The unit cost of in -patient service was 15,104.03 Bath per case or 531.45 Bath per hospital day with labor cost, material cost and capital cost per unit being 11,361.72, 2,800.33 and 2,125.24 Bath per case or 682.51, 133.57 and 126.24 Bath per hospital day, respectively. The operating cost per unit of in- patient service was 14,162.05 Bath per case or 816.08 Bath per hospital day. The total amount of out-patient service was 18,433 visit with full cost and operating cost per unit being 315.27 and 270.66 Bath per visit respectively. The out-patient labor, material and capital costs per unit were 242.76, 22.07 and 50.44 Bath per visit respectively. The full cost and the operating cost per unit of other mental health services were as follows: social work 183 and 163 Bath/visit psychology 459 and 416 Bath/visit dentistry 371 and 299 Bath/visit stress reduction 3,141 and 2,217 Bath/visit, rehabilitation 1,731 and 1,428 Bath/visit telephone counseling 398 and 375 Bath/call forensic psychiatry 614 and 509 Bath/visit mental health promotion and prevention 27,267 and 20,260 Bath/project. The study gave useful data for health economic analysis and brought this data for analysis of the cost recovery and breakeven point. This will be useful for hospital budget planning in the future.
