General physicians do accustom to the uses of isoniazid (INH) and rifampicin as the first line antituberculous drugs. Several untoward effects to both drugs (e.g.. Hepatitis, skin rash, hematotoxicities etc.) were often mentioned and also kept in mind. Suicidal attempts with massive doses of INH and rifampicin are uncommon. We reported a 27 year old male presented with coma, seizure and respiratory failure after taking 8 gms INH and 18 gms rifampicin. He was improved by various vigorous treatments of intravenous pyridoxine hydrochloride, hemodialysis and mechanical ventilatory support. As the resurgence of Tuberculosis in AIDS era, physicians should alert to overdoses of INH and rifampicin.
