Background: The aim of this study is to investigate the clinical manifestation for the cases of suicidal attempt between vascular dementia patients (VD) and Alzheimer-type dementia patients (ATD). Objects and Methods: We investigated 810 demented inpatients (300 men, 510 women) of 65 years old or more in Kotokukai-sato hospital in 20 years from 1982 to 2001. All inpatients were diagnosed from brain CT, the DSM diagnosis, and NINCDS-ADRDA. VD was 272 cases 33.7% and ATD was 445 cases 54.9% Results: In cases of 272 VD patients we found 11 suicidal attempts (VD-s;3 men, 8 women), and in cased of 445 ATD patients, we found 13 suicidal attempts (ATD-s; 2 men, 11 women), All the cases of suicidal attempts were in early of dementia. In the sex ratio, women were higher than men, especially in cases of ATD-s. The cases of VD-s have following features: patients with shorter morbidity period and milder cognitive disorder than the cases of ATD-s. The quality of familial communication and the chronic illness increase the risk of demented patients committing suicide. More than 20% of suicidal attempters had depression in their medical history. Conclusion: In the early stage of VD and ATD patients, anxiety hypochondria and paranoia, especially mood disorders in their medical history closely related to their suicide. It is important to note them for the prevention and reduction the risk of the suicide attempt.
