Objective: To reduce suicidality in first-episode psychosis patients attending EPPIC. Methods: Clinical audit and surveys demonstrated the prevalence of suicidality at EPPIC. A monitoring system was introduced to identify those at highest risk and those patients were referred for Life SPAN therapy. The effectiveness of the therapy was evaluated in a randomized controlled trial. Results: 56 suicidal patients with first-episode psychosis were randomly assigned to standard clinical care or standard care plus Life SPAN therapy. 42 patients completed the intervention. Measures of suicidality were taken before, immediately after the intervention and 6 months later. Benefits were noted on indirect measures of suicidality. The treatment group showed a greater average improvement (non-significant) on a measure of suicide ideation. Risk was highest within the first month of contact with EPPIC, it then decreased before rising again in month 5 and remaining high until month 16. Conclusion: Early intervention in psychosis for young people reduces suicide risk. Adding a suicide prevention therapy to this may further reduce risk. Patterns of risk vary over the course of treatment and have implications for service delivery. The findings from the study are now being integrated into a clinical program and a practitioner manuals is being developed.
