Objective: To determine the standard of psychiatric consultation reports for medically ill in-patients focusing one records of mood state. Suicidal ideas, and alcohol and substance abuse. Factors associated with such standard were also identified. Methods: Records and psychiatric consultation reports of patients admitted between 1999 and 2000 were sought and reviewed. Demographic data, as well as data regarding physical illnesses and psychiatric consultations were extracted using record forms developed by the investigators. Factors associated with records of mood states, suicidal ideas, and alcohol and substance abuse were also analyzed multiple logistic regression. Results: Rates of "failure to record" mood states, suicidal ideas, and alcohol and substance abuse were high (30%, 35%, and 38%, respectively). Psychiatrists had a tendency to overlook recording mood states of male patients than female patients, which accounted for approximately 60% of the cases. Likewise, they tended not to record patients with organic mental disorders when compared to those with functional disorder, also about 60% of the case. The rate of "failure to record" sucicidal ideas was also higher in male, about 50% (adjusted OR=3.1; P=0.007) and higher in patients with organic mental disorders. The rate was increased as the age increased. As for alcohol and substance abuse, physicians tended not to complete the records of more female patients than those of male patients (Adjusted OR=1.99, P=0.006), of older patients (p=0.02) and of patients treated in other wards when compared to those treated in the general medicine ward. Conclusion: the rates of failure to record mood states, suicidal ideas, and alcohol and substance abuse are still high among physicians. Patients' gender, age and diagnosis have significant influences on the failure to record such information. Physicians should concern more about patients' mood states, suicidal ideas, and alcohol and substance abuse.
