Objective: To investigate the changes of diagnoses form functional psychiatric disorder to psychiatric disorder due to general medical conditions among inpatients at the Department of Psychiatry, Siriraj Hospital, in a three-year period and to determine the factors related to such changes. Menthod: Data were collected from the medical records of the inpatients whose diagnosis had been changed to psychiatric disorder due to general medical conditions based on the ICD10 criteria upon their hospital discharge. Data collection took place from January 1, 1998 to December 31, 2000. The total number of inpatients was 29, and the subjects were eight patients who were diagnosed upon admission as having a psychiatric disorder. Results: It was found that of the total of 500 patients, eight of them had their diagnosis changed, accounting for 1.6%. Among these, the initial diagnoses were Bipolar l disorder 62.5% schizophrenia 25.0% and acute psychoses 12.5%. The factors associated with such changes prolonged hospitalization due to unresponsivensess to treatment leading to review of diagnosis and additional laboratory examination. Also, it was found that the patients had history, signs, and symptoms that indicated physical illnessess since admission. The most common diagnosis was epilepsy. Conclusion: Unresponsiveness to treatment of inpatients who were diagnosed with functional psychiatric disorder upon adminnion was an important reason leading to additional examination and subsequent changes of the diagnosis to organic mental disorder.
