Objective: This study was aimed to introduce a model of cost-utility analysis by comparing lithium monotherapy with lithium plus valproate therapy in treating patients with bipolar disorder Method: Published studies regarding the crucial parameters data from Phramongkutklao Hospital and data from leading textbooks of psychiatry were reviewed The decision tree was developed to compare the costs of treating with bipolar disorder for a period of one year. The outcomes were measured in terms of utility unity and cost-utility analysis (CUA) was conducted Finally sensitivity analyses by varying the effects of the response rates and mean survival times of the two regimens were performed Results To completely cure the patients with bipolar disorder lithium monotherapy costs more than lithium plus valproate which could save costs by 25% According to the sensitivity analysis lithium plus valproate is always more advantageous that than lithium monotherapy as long as the response rate is higher than 1.65% and the mean survival time is longer than 5.13 months However lithium provides equal cost effectiveness to lithium plus carbamazepine if the response rate is equal to 77% and the mean survival time is 5.60 months Conclusion: The results of this economic evaluation suggest that the combination of lithium with valproate appears to be more cost-effective than the use of lithium alone in the treatment of patients with bipolar disorder
