This article presents potential use of hospital electronic database in an analysis of variations in drug use across patient payment schemes as a tool for the Universal Health care coverage policy evaluation. Major sources of data included computerized databases for itemized drug dispensing and diagnosis as well as outpatient visits and inpatient admissions that have been recorded in five general and regional hospitals dring fiscal years 1999-2001. Characteristics of the database structure and approaches to data management were explained accordingly. Basis of the target analysus was classified into 4 patient subgroups: 1) patients receiving the drugs of interest; 2) patients receiving any drugs belonging to the same class as products of interest, 3) patients diagnosed with diseases that were indicated for the study drugs, and 4) patients admitted with the diseases of the study drugs. This article demonstrated certain analyses fot he hospital drug utilization data. Various quantification techniques were presented fro 1) utilization and expenses of drugs not covered by the National List of Essential Drugs, 2) use of epoetin and antiretrovirals in outpatients diagnosed with chronic renal failure and HIV infection; and 3) use of angiotensin-converting enzyme inhibitors and beta-blockers in patients with post-myocardial infarction and heart failure. Experiences gained from management and analysis of the hospital electronic databases showed that success in health indurance information system could be achieved through the development of standard set of electronic databases. Drug dispensing data based on the common coding system for drug products will facilitate comparison of drug utilization across hospitals. Major health care financing schemes including civill servant medical benefits, social security, and university health care coverage should agree on standard data sets to be used across the health insurance plans.
