Narcotics can destroyed a nation. The Royal Thai Government try to prevent and tackle the problems intensively without apparent success. Several factors, including limited information contributed to this failure. In order to establish a narcotic epidemiological pattern in the upper north, the researchers compiled all narcotics analytical record of laboratory examination of urine and samples of the Regional Medical Sciences Center, Chiang Mai requested by control authorities from B.E. 2527 to 2544. The results revealed that the number of urine samples increased progressively. The narcotics profile could be divided into 2 phases. First phase (B.E. 2527-2534): Heroin Era; general people and hill tribes predominantly were involved in the problems. There were some requests for glue analysis once the government lauched an Emergency Decree on Controlling the Use of Valatile Substabces B.E.2533. No request for tetrahydrocannabinoids analysis during this phase. An implementation of test Kits were very limit. Therefore, positive cases in a confirmation process were low. Second phase (B.E. 2535-2544): Amphetamines Era; at the beginning of this phase, the major substance found was amphetamine and then methamphetamine emerged in B.E. 2537 and dominated the scenario ever since. Drug abusers also shifted from drivers to school children, college students and blue-collar workers. There were some requests for tetrahydrocannbinoids analysis. Once all kind of test kits were well applied, positive confirmation rate was relatively high. It was Clearly shown that the narcotics problems are dynamic in terms of substances used and abusers. It is suggested that an effective operating procedure, good practice of drug testing urine sample handling and operating procedure for narcotics analysis should be standardized and well established. As such, the information system will be enhanced and more reliable.
