This study was aimed to assess first year implementation of the Universal Civerage Policy, namely 30 Bath Co-payment Scheme, in the first and second pilot provinces; Samut Sakhon, Phuket, Sukhothai, and Ubon Ratchathani focusing on management of health insurance benefits. The study was conducted during November 2001 to January 2002 by in-depth interview and focus group disxussion with provincial health administrators and managers providers (managers and professional groups), community leaders and beneficiaries. All groups recognized the policy was politically launched therefore sustainability of the project would be depended on political security and national economy. Various groups still preferred the scheme to be a safety net rather than public entitlement. Depending solely on government budget, they were unsure of financial sustainability of the scheme. Other policy components rather than providing insurance coverage, financing management reform and strengthening primary care, were perceived as insinuated by government officials and became critical. Management capabilities as crucial factors for success were problematic in all implemention levels. Time constraint together with lack of appropriate logistic supports and policy communication affected provincial implementation. Lack of good management capabilities and inappropriate provincial organizational structure were also main constraints. To improve the achievable management, purchaser-provider split, improving organizational management capabilities and training are recommended.
