Objective: To study the incidence of abnormal thyroid function due to lithium usage in Thai patients who were treated at Songklanagarind Hospital and to explore associated factors. Method : Medical records of patients who first received lithium during the period from 1 January 1998 to 31 August 2003 were retrospectively reviewed. Results : There were 355 patients who first received lithium, during that period. The 286 patients (80.6%) had been found medical records. Only 73 patients had the thyroid function test (TFT) prior to beginning treatment with normal reports. Finally, 10 of 73 patients (13.7%) had an abnormal TFT after receiving lithium. Half of them had abnormal TFT within 1 year of treatment and 90% of them within 3 years. For those who had abnormal TFT, 7 of 10 patients were fermale, 8 of 10 patients were less than 50 years old 3 of 5 patients who had abnormal TFT within 1 year were female 2 of 5 patients were older than 50 years old and 3 of 5 patients had nearly the maximum level of normal range for TFT prior lithium treatment. Conclusion : An abnormal thyroid function test due to lithium used was found to be rather common in Thai patients and could occur within first year of treatment, Due to some limitations of small samples in this report, the authors propose a prospective cohort study with a larger sample size for better information.
