This study was aimed to assess oral health implementation after the first year of the University Coverage Policy started on 2001 in five provinces; Chiang Mai, Nong Khai, Nakhon Sawan, Nakhon Pathom and Nakhon Si Thannarat which were purposively selected according to provincial oral health management and diversities of contracting units for primary care (CUPs). The study was conducted during September to October 2002 through a series of in-depth interview and focus group discussion with provincial and CUP managers and dentists. The CUP managers realized that their CUPs had to be gradually developed therefore the political stability in steering the concept and the policy were an important determinant of the implementing direction. Additionally, provincial managemment should be neutral and free from interferences. The main problem of the implementation was the oral health care delivery itself, a dilemma of an attractive benefit. Lack of capabilities at both provincial and CUP levels as well as impractical supportive measures at CUP level, the service were overloaded. To expand public oral health service depended on the work system improvement to respond to beneficiaries need, demand of service resources and accessibilities. While private dentists advocated fee-for-service with ceiling. They may increase accessibility very well through the 30 baht payment per visit rather than increasing physical accessibility at primary care units. For achievable result. Some recommendations on particular management and supporting system, the benefit and public-private mixed on preventive service for urban children were proposed.
