The objective of the prospective study was to determine the frequency, cause and severity of medication errors in internal medicine wards. All patients admitted to medical wards of Buddhachinaraj Phitsanulok Hospital during March, 2002-February, 2003 were recruited in the study. A protocol for detection and reporting was set up by physicians, clinical pharmacists and nurses. Medication errors included in the study were three types: prescribing, dispensing and medication adminstration errors. Causes and level of severity of medication errors were also evaluated. Physician orders, nurses kardexes and drugs in one day dosing medication cart were reviewed for prescribing errors and medication administration errors by clinical pharmacists, whereas dispensing errors were recorded by nurses. Medication errors were redressed immediately when they were detected. The total medication errors were in 5,040 episodes (7.94%) out of 63,455 prescriptions. Prescribing errors were in 1,706 episodes (2.69%) dispensing errors in 466 episodes (0.73%) and medication adminstration errors in 2,868 episodes (4.52%). The most common errors were unspecified strength (25.56%), over dispensing (40.56%) and omission error (77.82%). Personal error was the most common cause (64.98%). The other causes were improper transcription, excessive workload etc. Msot errors (71.92%), possibly harmful to patients, were corrected by clinical pharmacists and nurses before administering to patients. The medication errors reported by clinical pharmacissts and nurses could be beneficial in improving the medication use process in the hosiptal. However, further study is needed to evaluate the impact of health care team interventions, such as education and computerbased interventions, in order to lowering the frequency of medication errors and identify cause of medication errors.
