The object of this study was to determine drug interactions in out-patient prescriptions at Thanyarak Institute on Drug Abuse. The retrospective study of 2 or more items-precription were conducted from 1st October to 31st December 2002. The result showed that 1,204 of 3,880 prescriptions 31.03% were identified as having drug interactions. There were 59 paires of drug interactions with 1,930 episodes. The rate of drugs interactions trend to be increasing by the number of items in prescriptions. The mechanism of drug interactions were generated by drug metabolism process 44.07% and additive pharmacological effects 35.59%. The first 5 paors frequently found were phenothiazines-trihexyphenidyl, haloperidol-yrihexyphenidyl, tricyclic antidepressants-phenothiazines, tricylic antidepressants-trihexyphenidyl and mianserin-benzodiazepines. According to the severity of drug interactions, most were at moderate level 63.32%. Only 2 pairs of drug (propranolol-haloperidol and isoniazid-rifampin) were at major level of severity 0.67%. Some drug interactions may be harmful, therefore avoid the combination and choosing an alternative drug were recommended. If necessary, drug counseling and frug monitoring system should be implemented to minimise any potential adverse effects.
