Introduction: Department of Mental Health had developed a nationwide suicidal prevention programme since 2001. This study would explore more details in epidemiology of self-harm behaviors. It aimed at providing more understanding of the issue to help establish a comprehensive for the patients. Objectives: To explore physical, mental, socio-economic status, reasons for living of survived self-harm patients and to identify factors related to ideation of repeated self-harm. Methods: Cross-sectional survey was performed by randomized survived self-harm patients in 25 provinces and obtained patient preliminary data from 506 DS report during 1 January-31 December 2004. The total 1,200 patients were interviewed using structured questionnaire. Results: Response rate was 95% but completed only 718 cases (59%). Most of the patients were female, aged 20-29, married, laborer or farmer and students. Patient needs was idenfified from both of the patient and provider's perspective. It was shown that the top three problems which needed help and care were "job and income" (24-32%), "mental health" (14-19%) and "physical health" (15-17%). Regarding the self-harm methods, intentional self-poisoning by and exposure to medicine and pesticides were used in 45% and 40%, respectively. The main reason of self-harm was "having conflicts with the close ones" (45-49%). It specified which in a spouse or soul mate (39-49%). Most of the patients had no plan before intentional self-harm (48%). Considering to the reasons for living, it was founded that most patients (86-96%) reported they have to alive because of getting worried about their families and don't want to make their families cry. More than 60% of the patients had a life plan which mostly focused on job security, studying, and spending time with their families. Risk and protective factors related to ideation of repeated self-harm was analyzed by logistic regression. It was shown that mental disorder and depression were risk factors in female. In male, the study found that mental disorder and intention to die were risk factors. The higher reason for living scores, the more protective factor was shown, in both male and female. Conclusion: At present, there were 1/3 of self-harm patients who need helps in job security to have more income and comprehensive care in mental disorder and physical health. In addition, this structured questionnaire should be promoted to implement as routine services for self-harm group because it could improve the efficiency of mental care through the evaluation of self-harm ideation, behavior, and identification of specific interventions.
