รายละเอียด / Details:

The purpose of this research and development were to develop primary nursing service model for schizophrenic patients having violence behaviors. Research process consisted of three phases: 1) the development of primary nursing service model, 2) A quasi-experimental research, two-group pretest posttest design, to determine the effects of the model on self-care ability, anger management ability and nursing service satisfaction of patients, caregiver burden, and work satisfaction of nurses. 3) The revision of the primary nursing service model for schizophrenic patients having violence behavior. Research samples composed of 32 schizophrenic in patients, equally divided into one control and one experimental group by match paired technique, 16 family caregivers, and 7 staff nurses. Data collection was done by using the following instruments: complication and injuries from physical restraints checklists; patients' nursing service satisfaction and nurses' satisfaction questionnaires; and tests designed to measure self-care ability, anger control ability, and caregivers, burden. The reliability of two questionnaires and three tests were .95, .82, .70, and .93 respectively. Descriptive statistics and t-test were used in data analysis. The major findings were as followed: 1. The percentage of schizophrenic patients having violence behaviors in experimental group implementing the primary nursing service model was physically restrained less than those in the control group (18.75% vs. 60.50%). Moreover, there was no incidence of injury and /or complications among patients in both groups. In addition, patients in the experimental group reported their nursing service satisfaction at the above expectation level and at the expectation level 42.97 and 41.59, respectively. 2. The anger control ability and self-care ability of schizophrenic patients having violence behaviors in the experimental group, after the experiment, were significantly higher than before the experiment, and higher than that in control group, at the .05 level. 3. There was no difference between the burden in caring for schizophrenia patients among caregivers in the experimental group and the control group. 4. Nursing technology regarding primary nursing service model, consisted of Clinical Nursing Practice Guideline, Nursing service Manual for Primary Nurses and Related Nursing Personnel, which had been developed, tested, and improved was suitably implemented as a nursing practice model for schizophrenic patients having violence behaviors in which the nursing service quality, i.e. no complications, increasing patients ability to perform self-care and to control their anger, and increasing nursing service satisfaction among patients, could be expected. Moreover, with the systematically planned implementation of the newly nursing service model would have positive impact on satisfaction among nurses providing care for schizophrenic patients having violence behaviors.
