รายละเอียด / Details:

Depressive symptoms are dramatically increasing in Thailand. Persons suffering from such symptoms often loss their quality of life and become burden for their family and society. The purpose of this study was to explore health care services for persons with depressive symptoms in there parts: access to care, resources, and competency of health care providers. Qualitative research design was employed. Data were collected from various settings including a general hospital, community hospitals, and primary care units. Following verbal consent from participants, interview or focus groups was used to gather information from 5 physicians, 24 nurses, 18 other care providers, 82 village volunteers, and 17 patients with mental illness. Results: The results howed that for "access to care" clients had to follow the formal health care service system in order to be covered by the universal health care coverage (30 bath policy). Yet, problems were present such as inadequate services available at the community levels, transportation and financial difficulty. Regarding "resources" (policy, budget, and technology and materials), there was no specific policy for depression. The policy mainly focused on attempting to decrease rate of suicide. Budgets were received from several sources, but no specific budget for mental health services. Lastly, regarding technology and materials, though instruments for screening depressive symptoms existed, they were not properly used. In addition, limited choices of antidepressant drugs were also a concern. Related to competency of health care providers, general physicians felt unconfident to diagnose and provide treatment for persons with depressive disorders. Most nurses lacked skills for detecting depressive disorders. Conseling was the service nurses normally provided for persons with mental health problems. Other psychosocial interventions were not considered. Finally, village volunteers were received certain training in supporting risk groups for depression, but not in detecting depression. In conclusion, since depression is intangible is intangible and perceived bu most people as normal feeling, persons with depressive symptoms are often overlooked. Providers thus need more trainig to enhance their skills in detecting and taking care of persons with depression. In addition, making mental health services available at the community level could enhance quality of life of persons suffering from depression.
