Objective: To review the of olanzapine as a mood stabilizer for the treatment of bipolar disorder. Method: A Medline and Pubmed literature search of papers was conducted using the names of olanzapine and a number of key terms relevant to bipolar disorder. Data pertinent to the objective was reviewed according to the various phases and subtypes of bipolar disorder. Results: The data is substantive for the use of olanzapine in both acute and long-term treatment. In treatment of mania, olanzapine is effective in all patient subgroups, e.g. euphoric mania, mixed/dysphoric mania, psychotic mania, and mania with rapid cycling course. Olanzapine-fluoxetine combination is more effective than olanzapine alone or placebo, and olanzapine alone is more effective than placebo in the treatment of bipolar depression whether the course is rapid cycling or not. Olanzapine has strong evidence supporting its roles as potential long-term mood stabilizer to prevent recurrence in bipolar disorder of both manic and depressive episodes compared with placebo and is found to be as effective with mood stabilizers (lithium or valproate) have demonstrated significant beneficial effects compared with lithium or valproate monotherapy in treatment of acute mania. Long-term use of the combination of olanzapine plus lithium or valproate prolongs the time spent in symptomatic remission of olanzapine plus lithium or valproate prolongs the time spends in symptomatic remission compared with lithium or valproate monotherapy. The most common adverse events are somnolence, sedation, and weight gain. Conclusion: Olanzapine plays an important role in the acute treatment and maintenance treatment of bipolar disorder, both in monotherapy and combination therapy regimens.
