The Quality Assurance Programme has been established in the Ministry of Health Malaysia since 1985 and has succeeded in making the issue of quality and safety a national priority. The main objective of the programmer is to ensure that the patient family and community obtain the optimum achievable benefit from the services of the Ministry of Health within the available resources. Thus there was a need to establish a systematic approach to monitor and assure the quality of services. National Indicator in Psychiatry is one of the approaches of the Quality Assurance Program of the Ministry of Health It is intended that the quality of care provided is objectively measured and monitored and monitored against continually improving standards Shortfalls in Quality are identified and remedial measures are instituted. There are 4 National Indicators for Psychiatry which include. 1. Readmissions within 6 months of last discharge. 2. Defaulter rate of psychiatric patients attending psychiatric outpatient clinic. 3.Rate of psychiatric admissions of patients under hospital community psychiatric services. 4.Unnatural death. A comprehensive approach which involves hospital community psychiatric services outpatient psychiatric services rehabilitation in psychiatry and regular mortality audit were implemented to ensure the quality of care being maintained in our hospital and comply with the national standard.
