Objective: to develop a new tool for mental illness screening test in Thai community and test reliability and validity by compared with psychiatrists' interview diagnosis using M.I.N.I. 5.0.0 Thai Version. Methods : Tool development and diagnostic test study design had been done during 1 April 2001-31 May 2003. The study included the first phase of tool development. Item selection based on GHQ, MHI, TMHQ, Psychosis screening test by using nominal group technique (modified). Content validity was to determine concurrent validity by comparing this screening test with psychiatrists" interview diagnosis using MINI 5.0.0 Thai version. The 405 subjects aged above 15 years old living in the 4 regions of Thailand were recruited to answer this screening test and had been interviewed by trained psychiatrists simultaneously. Results: The Thai Mental illness screening test (TMIST) was the structured interview questionnaire. It consisted of 34 items within 3 parts. The first part consisted of 17 items asking about depression and anxiety symptoms. There were 6 pages of Likert, scale of severity of symptoms. The tool was presented to the subjects for choosing the answer (0 as none and 3 as almost always occurred). High internal consistency (Cronbach's Alpha 0.915). The second part consisted of 7 items asking about psychotic symptoms. Each items has dichotomous answer (0 as absent, 1 as present). Internal consistency was moderate (Cronbach's alpha 0.629). The time to use this tool was short (mean 6.88 (SD4.15) minutes). These two parts had optimum cut-off point in male was ≥6 with sensitivity 83.3% (95% CI, 72.3-94.4) specificity 57.9% (95% CI, 43.3-72.5) while cut-off point in female was ≥8 with sensitivity 88.6% (95% CI 85.1-92.1) specificity 64.8 (95% CI, 59.5-70). The final part consisted of 10 items of alcohol use disorder identification test. Internal consistency was high (Cronbach"s alpha 0.929). The optimum cut-off point in male was ≥8 with sensitivity 76% (95% CI, 66-86.1) specificity 70.5% (95% CI, 59.7-81.2) while in female was ≥6 with sensitivity 72.7% (95% CI, 68-77.5) specificity 88.5(95% CI, 85.1-91.4) Conclusion: TMIST had been developed at the first time and criterion validated among 405 subjects. Middle aged females, married and high stress were predominating. It had high reliability, high validity and easy application in primary health care setting. However, this tool needs more future research to evaluate validity among young female and male population.
