The author has reviewed literature on brain mechanisms that are involved in the substance dependence. Evidences indicate that mesolimbic dopamine system, also known as brain reward circuit, is responsible for the reinforcing effect of the drugs of abuse and chronic exposure to drugs may lead to both functional and structural changes in neurons of this brain system. These long-term changes are believed to be related to craving and compulsive use seen in chronic substance abusers. Also, genetic factor ( especially genes that control dopamine transmission ) might contribute to vulnerability to drug addiction. However, definite gene defects and exact mechanisms are still unknown. Further study needs to be done in the future with the goal to provide “ cure ” to clients.
