At the present time, there are many strategies to detoxify heroin addicted patients in multi-setting. Methadone maintenance remains the best-established pharmacologic treatment for heroin dependence in spite of its limitation. The rehabilitation program should be provided in conjunction with methadone treatment. The advantage of LAMM, a new opioid agonist, over methadone is that its long half-life eliminates the requirement for daily dosing. Buprenorphine, the partial mu-opioid agonist, has been studied for the treatment of heroin dependence both in detoxification and maintenance phase. Clonidine has been found to block effectively the autonomic hyperactivity of heroin withdrawal so it is a non-opioid medication for heroin detoxification. Lofexidine has fewer side effects than clonidine especially hypotension. Naltrexone, an oral opioid antagonist, is useful in maintenance phase of heroin abstinence but the major problem of treatment is poor compliance. The rapid opiate detoxification is a new strategy for shortening the opiate withdrawal period. Anesthetic techniques and clonidine have been used in this method to minimize the severity of withdrawal symptoms induced by opioid antagonists such as naloxone, naltrexone. The standard method of the rapid opiate detoxification needs to be further investigated.
