Electroconvulsive therapy (ECT) has been used to treat schizophrenia since its inception in 1938. At the present time, there has been no methodologically accepted studies documenting the efficacy of ECT in such patients. As a result, the role of ECT in treating schizophrenic patients in still controversial. The author review literature of all previous ECT studies, conclusions on their methodological flaws and the efficacy of ECT, and, the characteristics of an ideal ECT study. In 1994, the author conducted three pilot studies to address some of the problems in research methodology, i.e., the ‘hypothesized’ screening method, and the clear-cut assessment criterion for response and relapse. These features had been incorporated in the design of this study. One hundred fourteen patients with treatment-resistant schizophrenia (TRS) received acute treatment (Phase I study) with bilaterial ECT and flupenthixol. Fifty-eight patients met criteria, including clinical stability over a 3-week stabilization period, and were eligible for the continuation treatment study (Phase II). Fifty-one patients enrolled in the single-blind Phase II continuation trial and were randomized to three treatment groups. Treatment I-received ECT alone, Treatment II-received combined ECT and flupenthixol, and Treatment III-received flupenthixol alone. The duration of the study was 6 months. Combination treatment with ECT and neuroleptic therapy is much more effective in treating patients with TRS. This is the first systematic study to document the therapeutic efficacy of ECT in schizophrenia.
