Olanzapine is a thienobenzodiazepine derivative which displays efficacy in patients with schizophrenia and related psychoses. It has structural and pharmacological properties resembling those of the atypical antipsychotic clozapine and an improved tolerability profile compared with the classical antipsychotic drugs. Objective The purpose of this study was to evaluate the efficacy, tolerability and safety of olanzapine in Thai schizophrenic patients. Method In an open label study, 19 patients with schizophrenia (DSM-IV), who meet the inclusion criteria (CGI score at least 3, 18 to 65 years of age, no serious unstable medical illnesses), were assigned to receive olanzapine for 6 weeks after a washout period of at least 2 days. Doses were started at 10 mg/day for at least 5 days and then adjusted according to each patient’s response between 5-20 mg/day. The mean dose was 14.20 ? 2.98 mg/day. Treatment efficacy, safety and tolerability were evaluated with several well-known rating scales and blood chemistry. Results Olanzapine significantly reduced the severity of psychotic symptoms (scores on CGI, BPRS, PANSS) from the baseline. At the end point, all of the subjects clinically improved (reduction of 40% or more in total PANSS and reduction of 43.78% in CGI). Olanzapine appeared to have side effects only in weight gain and drowsiness; and its severity was generally mild. Neither reported extrapyramidal side effects. Three cases reported liver enzyme changed, but neither were significantly changed in clinical features. Conclusion Olanzapine was well tolerated and effective in both positive and negative symptoms in Thai patients with schizophrenia.
