Objective: To study the association of factors affecting medication nonadherence in schizophrenic inpatients including patient factors (age, sex, education, substance usage, alcohol usage) enviromentfactor (caregivers) and treatment factors (frequency of adverse drug reaction scores duration of illness frequency of drug administration number of drug regimens type of antipsychotic drugs). Method: This research was a prospective study. Results: One hundred and eighty-three of schizophrenic patients admitted at Suansaranrom hospital during December 2006 to February 2007 were enrolled in this study. Seventy-one percent were malepatients mean age was 36.77 +- 10.58 years average duration of illness was 9.43+- 7.94 years. Most of them received typical antipsychotic drugs (89.6%) where chlorpromazine was mostly used (63.9%). Patients also received antianxiety drugs anticholinergic drugs and mood stabilizers. Most of the patients received three drug regimens (38.8%) the frequency of drug administration was three times a day (63.4%) and 69.9% of patients did not have caregivers 56.8% of patients had medication nonadherence scores in low level (18.05+-7.49). Duration of illness and number of drug regimens were negatively correlated to medication nonadherence scores (p‹0.05) Male patients had medication nonadherence scores higher than female patients (p=0.022). Patients who did not have caregivers had medication nonadherence scores higher than patients who had caregivers (p<0.001). Patients who used alcohol had medication nonadherence scores higher than patients who did not use alcohol (p=0.02). Stepwise multiple regression analysis revealed that having caregivers (=-4.168;p<0.001) alcohol usage (=2.539; p=0.016) number of drug regimens (=-1455;p=0.004) and frequency of drug administration (=1.868;p=0.005) were significantly predictors of medication nonadherence in schizophrenic patients (R2=0.165;p<0.05). Conclusion: Factors associated to medication nonadherence found from this study can by implemented for monitoring on medication uses in order to obtain optimal treatment for schizophrenic patients.
