Objective: To study the long term natural course of the factors related to recurrence and functional status outcome of major depressive disorder patients treated at Songklanagarind Hospital. Method: A retrospective, descriptive study used systematical review of medical records from 1 July 2001-30 June 2004. Results: During the period studied there were 1,655 depressive disorder patients, according to ICD-10 criteria (F32 and F33), of whom 51 (6 males and 45 females) met the inclusion criteria of the study. There were a total of 192 depressive episodes in these 51 patients, of which moderate severity was the most common (52.9%). Severe patients had a higher rate of suicidal thoughts (77.4%) that the moderate severity ones (12.9%). By using multivariate Cox proportional hazard, it was found that factors that were associated with recurrent depressive disorders were sex, poor drug compliance, and severity of symptoms. Females were 6.5 times (95% CI=1.9-22.1) more likely to have recurrent episode than males while poor drug complaints were 4 times (95% CI=1.6-10.1) more likely to have recurrent episode than good ones. At the last partial disability were 11.8%, 43.1%, 33.3%, and 11.8%, respectively. Conclusion: Thai depressive disorder patients treated at Songklanagarind Hospital had a fairly good outcome. Female, poor drug compliance and more severity of symptoms in the first episode were associated with recurrent depression. Further prospective studies from various hospitals are recommended and will give us a clear picture of the natural course and outcome of Thai patients.
