Background : Health and social problem resulting from alcohol consumption are widely recognized. In Thailand, the number of adolescents who drink alcohol is increasing, especially in Isaan community. Objective: To study the initiation of alcohol consumption of adolescents in Isaan community. Material & Method: A qualitative study. Key informants are adolescents in sub-urban community, Khon Kean province. Data were collected through participatory observations, in-depth interviews and field notes. Triangulation of data was done to ensure the credibility and rigor of the study. Data were analyzed using content and thematic analysis. Results: Adolescents start to drink alcohol by: 1) encouragement from friends and family. Adolescents start to experience various cognitive and physical changes, it is frequently noted that they start giving greater importance to their friends. 2) Isaan religious ceremonies are connected to alcohol consumption. It was found that alcohol was always consumed before attending the ceremony for cheering up their spirits 3) Special like birthday, graduation ceremony, and finishing examinations. Adolescents might have a sense of 'grown up' therefore alcohol should be used for cerebration instead of soft drinks. 4) Living in apartments, neighboring houses, and grocery stores outside villages. These places are most suitable for drinking and hiding from parents. 5) Gang leader encourages alcohol consumptions to their members. Adolescents who want to join the gang must obey the leader so they were forced to drink. And 60 Beer and rice whisky were among the first beverages since they are cheaper and easier to purchase than other kinds. Conclusions: There are many ways for adolescents to start consuming alcohol. Primary health care professionals' family and community need to understand social and cultural context surrounded alcohol consumption among adolescents. They have to work together, they have to realize that it is their responsibility to prevent and solve the problem.
