Many studies have found a considerable impact of alcohol on health and non-health expenditures. Specifically, empirical evidence has consistently indicated that a major component of the social costs of alcohol is closely associated with the loss of productivity. To estimate the cost of productivity loss associated with alcohol, data on productivity loss associated with alcohol are needed. This study aims to compare productivity loss due to general health problem across alcohol drinking status in term of absenteeism and presenteeism. A cross-sectional household survey was carried out throughout the country. Thai populations, regardless of their drinking status from 4,330 households, whose age between 15-60 years and who were on paid employment, were interviewed in this study. Questions consisted of 3 main parts (i.e. general information, work impairment, and alcohol consumption). Questions concerning work impairment were adapted from the work productivity and Activity Impairment-General Health Questionnaire. This study indicates that alcohol consumption was significantly associated with productivity loss both absenteeism and presenteeism. The magnitude of productivity loss relied on the level of alcohol consumption. It was estimated that the reduction of overall productivity due to health problem were 13.9% (95% CI 11.2,16.5) among hazardous drinkers, 13.2% (95% CI 11.3, 15.1) among former drinkers, and 9.2% (95% CI 8.5, 10.0) among reasonable drinkers in comparison to 7.6% (95% CI 6.7, 8.5) among abstainers. These differences were statistically significant. Based on the pattern of alcohol consumption among Thai population, it was estimated the loss of 133,277,683 working days or the loss of productivity cost of approximately 30,000 million Baths in the year 2007. This result supports the policy and intervention aims to reduce alcohol consumption.
