Children who have critical or chronic pulmonary disease are associated with increased respiratory secretion retained or obstructed airways. The effect to children are suffering and morbidity (for example, reiteration, difficulty of weaning off oxygen, stay in the hospital for long time) and cause their family and parents’ anxiety and stress. Care these children are difficult and complicated. The using of traditional methods might not been effective enough to resolve their problems. The purpose of this action research is to take innovations from evidence to promote secretion drainage in children who have respiratory secretion retained or obstructed airways. The methodology to used 4 steps of evidence based nursing practice were 1) clinic situation analysis and to determine problem,2)finding evidence best practice, developed practice guideline, and to set outcome,3) take the practice guideline to pilot study and 4) evaluation and analysis outcome. Method include participatory observation, non-participatory observation, informal dialogue, sub-group meetings, finding evidence based practice, pediatrics patient care team consultants, specialist bedside learning and telephone consultants during October 2006-July 2007. Ten of children who were sampled by purposive sampling enrolled in this study age range 1 month-12 month. They had problems about copious secretion, and inability to expectorate, and admitted in Pediatrics Department Sakon Nakhon hospital during March –July 2007. Instrument include clinical respiratory assessment form, children behavioral observation form, satisfaction questionnaires, clinical practice guide line for secretion drainage by using EZPAPTM and MU TIP with suction. Descriptive statistic and content analysis were used. Results were evidence based practice and innovations benefit patients as follow: no evidence of re-intubations and readmission in NICU and in hospital, reduction of length of stay in hospital, improving parents and healthcare teams’ satisfaction. The conclusion, evidence based practice used to improved effectiveness of nursing care and promotes patient safety. Consequently it should continue and develop policy to improve quality of nursing care for other groups of patients.
