The National Burden of Disease (2004) reported the five leading burden diseases were: ischemic heart disease, mental illness, cerebra-vascular disease (stroke), road traffic injuries and cancers. Both the public and private sectors provide health care in Malaysia with the Ministry of Health acting as the lead agency. The health care I indicators indicated good pro grass with high life expectancy: low birth mortality and death rates. Mental illness was reported to be the second cause of disease burden in Malaysia. The vision and mission of the Ministry of Health, the vision for Mental Health, the Mental Health Policy and the National Mental Health Framework, guide the development of mental health services in Malaysia. The government facilities providing psychiatric care include four mental institutions, and twenty-six government hospital and 763 primary health care clinics . In addition twenty-five of these clinics also provided psychosocial rehabilitation services for patients with severe mental illnesses. Training of psychiatrists is done conjointly in a 4 year program me and there are now 70 doctors in training. There are 3 nursing schools providing post basic psychiatric nursing training with placement for 90 nurses. The future plans include expansion of future plans include expansion of acute psychiatric beds in the general hospital setting; providing more coverage under hospital based community psychiatric service which include acute home care (alternative to acute hospital admission), assertive community treatment and follow-up of ‘difficult’ cases; strengthening primary and secondary care services and reducing the reliance on the 4 mental hospitals. The challenges faced in Malaysia are: i)coordinating care among the different facilities and localities, ii ) having adequate trained manpower, iii) streamlining the involvement of NGOs and families in the after care of patients in a more structured manner.
