In in this presentation the aim is to review the present mental health status in Sri Lanka and the challenges facing the country in the coming decade. Both the emerging patterns of mental health burden and emerging service provider networks will be reviewed. Data is collected from the existing publications on mental health in ralation to morbidity patterns, clinical services, training programs, research, prevenyion and mental health organization of the Ministry of Health. There is evedence to suggest that apart from psychosis and mood dosorders the emerging conditions such as substance abuse, suicidal behavior, mental disability, behavior disorders in children and dementia are contributing significantly to mental health burden. Apart from these disorders, which are familiar to psychiatrist, mental health problems related to trauma, disasters, conflicts and social determinants such as domestic violence result in poor health. The mental health organization, public mental health care provider network, training, social care, rehabilitation and public education program have been slow to repond to emerging needs. The servicse continue to be based in institutions, urban areas and manned by medical personnel. The role of non-health public agencies such as social welfare, community based organizations and the private sectors have increasingly been visible in mental health care, however the coordination of these organizations with public mental health services has been minimal. In spite of significant growth of mental health services in the last decades there are still major concerns inrelation to access to services, quality of care, rights of patients with mental illness and the affordability of care. The paper discusses the challenges facing the Mental Health planners in the coming decade.
