Painful physical symptoms are common in patients with major depressive disorder and associated with more severe depression of longer duration, greater functional impairment, poorer clinical outcome and higer health-care costs. Painful symptoms in depression improve substantiallyduring the first month of antidepressant treatment, however patients with high baseline pain severity and less pain improvement over time are associated with worse depression treatment response. Insomnia is also highly prevalent in patients with major depressive disorder. Previous studies in pain patients have shown that sleep disturbance is positively associated with pain severity, even after controlling for the effects of psychological distress and behavioral factors. This presentation will provide an up-to-date review of the inter-relationship between pain, sleep and depression. We will also present our preliminary data on the predictors of painful symptoms at baseline of a depressive episode and three months after antidepressant treatment, and pattern of change of pain, sleep and depression variables.
