Atypical antipsychotics have some efficacy in treating bipolar depressino but the precise mechanisms underlyingtheir efficacy remain unknown . A sueveyof their receptor profiles suggests that they all bind to brain 5-HT2A receptors, In this regard, it is interesting to note that Positron Emission Tomography (PET) studies suggest that antidepressants that belong to different classes such as desipramine , paroxetine and nefazodone downregulate brain 5-HT2A receptors. As well, more recently our group has shown that electroconvulsive therapy also downregulates bain 5-HT2A receptors. These findings indicate that down-regulation of 5-HT2A receptors might be a potential mechanism that underlies the antidepressant effects. Tnterestingly, rodent studies suggest that administration of atypical antipsychitics also result in down-regulation of brain5-HT2A receptors. Hence, it is likely that this prooerty of atypical antipsychotics may account for antidepressant effects of atypical antipsychotics in bipolar depression. The other mechanisms might include neurogenesis, a property that is shared by many atypical antipsychotics. However, the magnitude of antidepressant effects appears to be somewhat greater for quetiapine compared with other atypical antipsychotics tested to date. More recent studies suggest that one of the metabolites of quetiapine is a potent inhibitor of norepinephrine transporter, and thus is expected to increase brain norepinephrine levels. It is currently believed that this aditional property of quetiapine may account for it's better antidepressant efficacy.
